Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are excited to share that we are looking for an ACP to work within our GP Practices in Primary Care. We have set shifts available over our two clusters around Merseyside.  


Salary: £25072.80 - £25748.21 per annum

Contract: 15 hours part time

Location: Various including 15 Sefton rd, Litherland, Maghull, Thornton Health centre & Crosby/Crossways

Shifts:  

Monday 10.30am-6.30pm (15 Sefton Rd) & Friday 8am-4pm (Litherland) (15 hours)

Wednesday 8am-4pm (Crosby/Crossways) (7.5 hours)

Wednesday 8am-4pm (Maghull) and Tuesday 8am-6.30pm (Thornton) (15 hours)


What will you be doing 

  • You will undertake comprehensive health assessments both face to face and over the telephone, including adults and paediatrics; practicing autonomously whilst demonstrating a person-centred approach to care delivery. 
  • Within your professional boundaries you will assess, diagnose, plan, implement and evaluate treatment/interventions. Provide care for patients presenting with undifferentiated and undiagnosed conditions and complex needs. 
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.    
  • You will practice autonomously and prioritise, organise and manage own workload in a manner that maintains and promotes quality.   
  • Take responsibility for own learning and performance, acting as a positive role model. 


What do I need   

  • Full NMC Registration with no restrictions 
  • MSc in Advanced Clinical Practice is desirable; candidates working towards this qualification or who have relevant experience are encouraged to apply. 
  • Non-Medical Prescriber – V300 
  • Clinical Examination and Diagnostics Level 7 
  • 2+ years of experience working within a GP Practice.
