Job Introduction
Do you have customer service and administration experience? Primary care 24 are looking Care Navigator – Performance to join our workforce, who will provide excellent customer service to patients, carers, healthcare professionals and other services when making calls on behalf of PC24 GP practices and do so in a caring and compassionate manner.
Salary: £25,499.00 per annum (pro rata)

Location: Primary Care Hub, Sefton Street, Liverpool
Shifts: Monday – Friday (08:00 – 16:00 or 10:30 – 18:30) Full time (37.5 hours) Fixed term contract until 30th September.
What will I be doing?
- Provide care navigation call handling duties on behalf of the PC24 practices, in line with PC24 processes and procedures.
- Undertake outbound call duties on behalf of our practices, undertaking basic questionnaires and arranging patient appointments.
- Ensure through effective communication that all calls are directed to the most appropriate clinician and/or service, identify and escalate any emergency callers to appropriate ambulance services.
- Request basic investigation (I.E blood tests) as instructed by healthcare professionals.
- Provide support to the wider primary Care team and action and follow up calls to other services as required.
- Provide operational support to the onsite practice team and clinical team.
- Record all essential information in a concise and accurate manner using EMIS Web system processes.
- Liaise with other services to request any instructions given by the clinical team, ensure all information logged.
- Respond to circumstances with a positive and flexible approach.
- Understand local referral arrangements/pathways for holistic support from a wide range of sectors.
- Ensure patient confidentiality at all times
- Be familiar with and adhere to PC24 Standard Operating Procedures.
- Be patient focused, ability to show patience, empathy and compassion via the telephone.
What do I need?
- Excellent verbal and written communication skills.
- Flexibility to work as a team and independently and be able to work across all PC24 sites, as requested.
- Public Health knowledge
- Experience of care navigation within QOF
- Great IT skills including all Microsoft package – experience of EMIS is also desirable.
- Previous experience in a call handling position
